You don't have to handshake if you don't want to or if your hand is injured. However, in business it certainly is customary to extend your hand at first meeting and again when leaving. Many people really are uncomfortable extending their hand. There was a period of time in my career that I was responsible for ensuring that the staff extended every courtesy to our clients and I was most surprised that they didn't know how to do it. We actually had to teach a lot of the staff when to extend their hand & how it shouldn't be limp or a fierce squeeze. It was not a custom in many of the cultures and Canada is a multi-cultural country. In my own family this was taught when we were children and still is down to our grandchildren. It is really sweet when you arrive at their houses and are greeted by a 3 year old with hand extended but that is the culture I was brought up in.I

It is normal. I think that handshaking works in most countries except for a few.

I don't think it is a required, but it is quite a normal part of business... even just casual introductions. I suppose the major germ-phobes would be against it, but a really poor handshake can set the tone for a meeting (badly...)
