Im trying to seperate the service by going to school, but Im not sure how im suppose to fill out my 4187. Does anyone know where I can get an example of a DA 4187 where its is used for seperation under the discharge for further education?

The second link is Army Regulations 600-8-6 that has instructions for filling out DA 4187. Read pages 5 & 6 and page 20.
The third link is a copy of DA 4187 that you must print out in order to fill it out.

The easiest way to get this information is to go to your Battalion Pac and ask the NCOIC for either the information or the Army Regulation dealing with such actions.
