Good quality bread and fresh ingredients. The rest is up to you. I've seen good interpretations and bad interpretations of many classic sandwiches. Most of the time, it's the freshness of the ingredients than can make or break a sandwich.
One thing that I like to do to virtually every sandwich is to toast the bread until golden brown. I add this as an afterthought, as I know that many people don't like toasted bread on sandwiches. Try it sometime.

I coat a thick cut bread with butter and sprinkle heavily with parmesan
Then I put good honey mustard, sun dried tomatoe flavored mayo on the bread.
Then in the sandwhich try thin deli smoked turkey, and deli style pastrami, cooked pieces of bacon, green pepper, with mozzerella and swiss cheese.
Then I put it on the grill until both sides are toasted and then through some lettuce in and YUM YUM!

A mouth watering sandwich is filled with items that you love; I always have hot or sweet capacola, buffalo turkey breast, maple, brown sugar, or honey ham, genoa salami, and new york or vermont extra sharp cheddar. I eat it on a dark german or russian, or 9-12 grain bread. I have mayo and honey mustard. I never deviate.

Crusty Italian bread, olive relish, tons of Italian meats, and provolone. I'd walk 3 miles in snow uphill to get a muffaletta. I'm drooling right now.

Iowa loaf, pepper jack cheese, sliced tomato, and mayo on whole wheat bread.
For something more substantial:
Grilled chicken breast, topped with swiss cheese and crisp sliced bacon, with vidalia onion dressing on a hogie roll
