USCIS suggest applicants to file at least 45 days before their visa expires (date on your I-94). you need your original I-94; written statement explaining in details the season of your request, departure arrangement, any effect the extended stay may have on your foreign employment or residency; and application fee US$300. which service center to file your application depends on your current address in US. please go to USCIS official website for details:

Hello. You need to file for an extension with the USCIS, stating the reason why you need an extension. You have to apply for this well before your current visa expires.

Eligibility Requirements: Anyone currently in the United States on a B-1 or B-2 visa may file for an extension of stay. However there are no guarantees that the application will be approved. The USCIS takes into consideration factors that show your intention of departing the United States at the end of the proposed extension period. The applicant must submit the extension of stay application before the current authorized stay expires. The applicant is also required to provide proper evidence of financial support.
Here is a link for the form from the USCIS website:

Good luck!

Request for an extension of the US visa at the Immigration and Naturalization Services of the US but cite valid reasons.
