q:1:-the weapon's injuries considered of what type??

q1 makes no sense. You are missing some information.

q2a: If a bullet penetrates say an arm then exits and then penetrates a chest then exits you will have two entries and two exits from one bullet.

q2b: same as above, but it does not exit the second time.

q2c: the bullet enters then breaks up before exiting in two pieces at two different sites.
