You could cut a stencil and use cocoa and powdered sugar to create a design - like coat the cake in a dusting of powdered sugar and lay the stencil on and dust with cocoa powder to create the design. Or use colored sugars for the design. You could use almond paste and roll it out and cut a top for the cake. You could use whipped cream or sweetened cream cheese. You could use fresh fruit. You could use melted chocolate as a base or design element. Also chocolate shavings and curls or toasted coconut are nice. The website below has info on stencilling and these edible food transfers that you can order and transfer any picture on a cake.

1) Fresh fruit in cream or cool whip
2) put a doily on the top of the cake and dust it with coco powder or icing sugar. Remove the doily and you have a nice pattern on top.
3) sugar flowers
4) fruit kabobs

You can buy characters at many craft stores for the top of cakes. Personally I have used little plastic toys I have cleaned first.
