This doctor has been my doctor for many years, now I'm moving out of state and would like to say a special thanks to him.

He was your physician and you and your insurance company have paid him very well for his services. So, I don't think you should give him anything monetary. He would appreciate a nice card expressing your gratitude for his warm care over the years. The office staff has also played a big part in your care as with scheduling, calling in perscriptions etc, so I think a nice plant for the office along with a box of chocolates sent to the staff thanking them also for being pleasant would be a very nice act of appreciation.

How about a nice card and a plate of holiday treats. I'm guessing his office staff and nurses have probably been there for years with you as well. A plate of cookies can be enjoyed by everyone at the office.

What about a gift certificate to a nice, local restaurant? Or a donation made to a charitable organization in his name, perhaps related to his practice's specialty)?

I don't know what to get him, but he will appreciate the less hassle involved if you have them copy your medical records to take with you instead of doing it via mail or internet.
