this should not be requested in a vacation form. It should be requested in person. It is not a small thing to replace an employee for 45 days and so your boss should have as much time as possible to fill in the missing slot. Especially since you have been promoted thus your responsibilities have most likely increased making your presence more valuable to the company. A 45 day leave of absense could very likely cause your boss to decide not to re-hire you. So, talk to him about your request and see what will work for both of you.

You must have a sympathetic boss to get 30 days off from work in your 2 year of employment. I think you are best off having a friendly chat with the "power that is" before writing small notes. The power may decide that if he can without you for 45 days, he can do without you all the time.

Ask for a leave of absence, not just "time off". This would be a sit down with a manager type thing.

Maybe explain to your employer why. Let him/her know that it's not just a vacation or whatever, but you're going to see family. I don't know if that would make a difference, but, good luck.

How do you ask for 45 days off of work? The same way anybody else does it when they quit!
Good luck!

I think that length of time would require a leave of abscense form
