Her husband who urged her to go off of her medication and have another baby after he had been warned by her psychiatrists that having another baby could seriously harm her mental state.
And partly the doctors that kept medicating and releasing her without calling Social Services to check on her children and make sure their home life stayed safe. If SOMEONE besides the husband had been told that this woman was unstable and a danger to herself and quite probably her children, someone besides her unspeakably selfish and STUPID husband may have been watching out for those poor children.

I would blame the woman. Her babies, were not given a chance to live because she took that away from them. She is not that crazy or insane, is she was why didn't she take her on life.
