If you mean a Canadian who is a US green card holder it is obviously quite simple.

If you are a foreign national who holds a permanent residence card in Canada it is very different. The Canadian Green Card just means you get to live in Canada. Your entry into the United States is dependent on your nationality. If you are a citizen of a Visa Waiver Country you may enter on that program, if you are eligible of course. If not you must obtain a visa to enter the US.

you mean a canadian permanent resident? they're not called green card holders... they're called maple leaf card holders. green card refers to US only, not canada.

it all depends on your citizenship. if your home country is part of the visa waiver program with the US, you only need your passport to enter. if it's not, then you need to go to the nearest US embassy or consulate to apply for a tourist visa.

as long as you have a stable job and pretty much is settled in canada... like you have enough evidence that you will not be overstaying your visa... then you have nothing to worry about.

do you have a valid passport? as long as you don't need any specific paperwork you should be fine. Your P/R card will allow you to re-enter Canada.
