I think flirting starts with sexual attraction and progresses with comfort. Think about how it begins. You meet someone. If they are not so good looking you might talk to them but you don't end up flirting with them. If you find yourself attracted to them and they are cute, hot or goodlooking you start flirting. That's usually how it begins. If they give back the same vibe and there is a level of comfort you keep flirting. If they give off a vibe, like get away from me, which is not so comforting you might walk away. Like I said flirting starts with attraction and flirting progresses with comfort.

Since flirtation is a decidable action, I would believe it would be up to the person. People flirt with who they like. The question is why they like them. If they are flirting just because the other person is pretty, then it would be sexual attraction.
But there are people, including me, who like the person for who they are and not what they look like.

Most certainly depends on the person doing the flirting.
some people are very flirtatious and do it all the time with friends. these are generally more outgoing people. these people are harder to read because most of the time flirting from them is just what they do, however that doesnt mean that they do anything other than what a normal person dose with someone they "like".
other, more shy people, reserve flirting for people they like. any type of flirting at any time from this person should be considered attraction.
the very flirtatious person is the one that you have to be careful with because you dont want to get the wrong message from them and make it awkward

An interesting question. I would say it's both,
yet distinctly separate. i.e. the young man who
flirts with the old woman to make her smile and
feel young again. This is a level of comfort and
not necessary attraction. But, two people of the
same age...I'd call it attraction.

Well I flirt with people who I am attracted to mostly. How ever there are those times I will flirt with some one to make them feel good about themselves.

It could also be they just want something & know if the "turn it on" they will probably get it.
