was it a piece of paper? was there a logo like Rx like you see these days? how did doctors give information to drugists?

There were apothecaries or druggists who did their own formulating. There were no controls in colonial days as there are today. Standards of medical education in the U.S. came only after the American Civil War. That was one of the goals of the AMA which was formed in the late 1840s to improve education and professional standards for medicine.
There were very few "prescription drugs" that actually worked in Colonial times. Opium as an alcoholic mixture known as laudanum worked for cough, pain and diarrhea - - but this did not require a prescription until the early 1900s. In Colonial times you could get any drug "over the counter." Cocaine was said to be part of the original Coca Cola formulation (which I believe is still a trade secret) before 1900.
Quinine for malaria, digitalis for dropsy (heart failure) and colchicine for gout were other efficacious meds when they were used correctly.

There were pharmacists back then. They did mostly compounding. Sometimes the prescription was verbally given or written down. The pharmacist would sometimes prescribe their own medications.

My best guess is that there were no pharmacies, the doctors kept all they needed in stock themselves. Either that or everything was OTC.
