1. Try not to lift alone, you can go alone just make sure someone else is there to help you.
2. Use a spotter, I go alone so I only use one if I'm trying something really hard, but never chance it. Most people at the gym would be happy to give you a spot.
3. ALWAYS, ALWAYS use collars! It drives me nuts to see guys trying to put up even a plate on each side of the bench bar without collars, it's stupid and you can really hurt someone if a plate slips off the end of the bar!
4. Use proper equipment, damaged or worn equipment can cause a great deal of harm to you or anyone around you, just 10lbs. moving at 50mph. 5ft. off the ground has a lot of potental energy that could be released if a cable broke or a plate fell off.
5. Take the time and learn it right!, don't speed through lifts, and if you are doing a new lift you need to have someone show you how to do it and coach you into form. Form errors cause many problems later in life, make the lifts inefficent for building muscle, and can be very unsafe, espically on full body lifts (squats, deadlift, and any kind of cleans)
good question and be safe!

The number one resource I found online that teaches how to gain muscles (pounds of muscles!) and shred your body fat is http://www.goobypls.com/r/rd.asp?gid=552.
it's the only one program that worked for me (and still working)
