a "diet" is nothing more than the regular food and drink of an organism. and yes you do have to be on a diet to gain muscle mass. building muscle is all about the diet, resistance training only stimulates the muscles and central nervous system. the foods that you ingest are what the body uses to repair cells and construct new ones. so the quality and amounts of the foods that you consume will have a direct effect on the rate at which you gain muscle mass. using progressive resistance breaks down the muscles and the diet repairs them and provides the building blocks for new cell structures.
to gain muscle mass you need to have a caloric excess in your diet. normally if you take your bodyweight and multiply that by 20 that will be sufficient calories for weight gain. if not then increase the cals by 10% each week until weight gain occurs. carbs are actually more important than protein in a mass gaining diet. if your body fat is low (14% or less) then 30% protein, 50% carbs and 20% fats should work. if your body fat is higher more towards the high teens then I would use macros of close to 40% protein, 40% carbs and 20% fats.

You don't have to be on a diet to build muscle. But if you have extra fat on your body, then you should diet while building muscle because otherwise the muscle that you build will be hidden under the layer of fat, making you look bigger but not "muscular" and fit.
If you are already thin or fit, then diet is not necessary since whatever muscle you build will be immediately noticeable. If you stick with a weight training plan and increase your protein intake, you should notice muscles bulking up within matters of a few weeks.
Adding protein to your diet, be it more meat or using protein shake supplements or meal replacements, will make a big difference if your goal is to build muscle FAST. And for faster results, you should increase your weights in weight training consistently, but be careful not to go overboard as it will be really bad for your back/bones.

No, work out with free weights is the fastest. Hi protein diet will help. Avoid fatty foods. You should do at least 3 x 15 reps and to know if you have enough weight to build muscle- the last 15 reps should be very difficult- almost like you can't do it. Do this every other day to let the lactic acid seep.

You need to eat a lot of protein to gain muscle, at least 1g of protein per 1lb of body weight (or lean mass) spaced out into 5 meals throughout the day.
