How much does a marriage license cost?
Does it vary by state (I live in Jeffersonville, IN)?
Will we be getting married by a judge?

Every state has different regulations and costs. You will have to schedule ahead of time since judges marry only at certain times and days. It is a short ceremony and you exchange vows. You need two witnesses.

Every state is different. In my experience city hall weddings are very cold and are just the bare bones to get the job done. I can get that covered in under 2 minutes. In NYC it's done through a hole in the plexiglass. However there's a Town Clerk on Long Island that marries town residents for free in the Gazebo next to her office one day a month. My advice is to call a professional wedding officiant and ask. Tell them you were thinking town hall but you don't know what it entails and you don't know what hiring an officiant whould be either. Call a few as you'll get different price ranges.

I live in VA. it cost $50 for the ceremony (marriage lawyer) then it costs $30 to get your license at the courthouse. They will mail your application to you within 10 days for you to mail back if you want copies of your license .. which is about $2 per copy. If you do not request a copy, they will only keep your license on file, you will not get any copies.
