Not sure I understand exactly but I'll give it a try. If it's YOUR birthday, and someone gives you a gift, card, etc., then all you would have to say is "thanks and God bless you" or something along those lines.
If it's someone else's birthday, you have more options. One, you can say "May God bless you on this special day" or something along those lines. You could give a religious-theme card, or even e-card (http://www.christianet.com is a good place to start--IF the other person has email!). You could make a donation to the Gideons or Gospel for Asia or The Bible League in the name of the birthday person and they'll send a recognition card to the person.
More practically, why not bake a cake and share with the person? There are a lot of ideas--I've only spun a few off the top of my head--but above all, trust the Lord to give you good guidance. May you and all of yours have the best Christmas ever!
