Under normal circumstances Fica is not reported on a 1040. An exception exists if you worked at more than 1 job and between all of your employers you earned more than the SS withholding maximum. In that case you report the excess SS withheld on your 1040, and it is treated like federal withholding tax.

Self-employed individuals pay into social security, but it is computed on Schedule SE. The amount owed is entered as self-employment tax on the 1040.

Usually it doesn't. If you have worked more than one job and that's resulted in social security being deducted beyond the limit, the excess goes on line 67 of a form 1040. Self-employment tax goes on line 58, and ss and medicare on tips not reported to your employer go on line 59.
