first of all this is all speculation on my part.
Things that help me might not help you. The most important thing I see is that you are stressed out because you are afraid you are "going to fail again." You need to let go of what has already happened and focus on what is happening now. In other words stop thinking about failing before you even try to succeed. Also tea helps me to concentrate on the task at hand, as does a little Chopin.

When you notice yourself wandering, bring yourself back. Work for 20 mins then take a 5 min break. You're going to have to make the commitment and do it, even if you don't feel like it. You could try doing relaxation techniques to focus yourself before starting. You can reduce distractions by making sure the tv/radio/etc. is off and there are not other people around.

Well, sometimes music helps. Also, it sounds dumb, but Vitamin Water's "Focus" flavor can help a lot with concentration. And, if you're afraid of failing, you should have quite enough motivation to just do the paper...you just have to realize how much of an impact it might have on your grade.

It's sort of like writer's block. You simply are not interested enough in that subject to write about it. It is not that uncommon really, maybe go for a walk, watch a movie anything that distracts you from the problem for the short term. Before you know it, you'll be done and you can move on...

I have issues concentrating to.... Too many distractions. I find the only way to make myself really get on with the work is to sit at a desk with nothing on it in an emptyish silent room. Make sure there is nothing entertaining in there. Then take a break every 30-40 mins. Alternatively fiddle with sumthing like bluetack while u think,

Try doing an outline- as you do it you should come up with creative ideas, and you'll get excited about it.
Or make a list- break up the paper into bits. Again, if you come up with an original idea, you'll get excited and want to work on it.
Don't start with the intro paragraph- that will just stump you.
Sometimes I promise myself rewards as I finish each bit of the paper.

I would see how much you could write about the topic in say a minute, and see how many lines of paper you took up and try to beat you record over and over until your finished.

sounds like the instructions are no longer tricky on your skills. although, numbness is a factor of existence - like doing dishes 12 months a twelve months for sixty 5 years. that is needed; do it and get on with issues you delight in. So, do the uninteresting - just to get it out of ways - then do the flaws you particularly delight in! As for historic past, discover a dork, ask him/her for tutoring.

Think about what you want and how you are going to acheive that. If doing well on this paper is important in acheiving your goals then it shouldn't be hard to do.
Manage your priorities. I've been where you are many times and this always helps.
