My wife have a passport with her last name. After marriage she didn't change her last name and added spouse name. so what i wan to know is can she go and apply for F2 on that passport or she have to change her last name and spouse name.

In case her passport has your name as spouse added & your passport has her name added as spouse, she can be applying for F-2 Visa. There is no need for her to change her name or surname for this purpose. The other documents along with this passport are your valid marriage certificate issued by the marriage officer/registrar, her birth certificate issued by the local municipality, her police verification report from the age of 16years till now & medical report. All the certificates/degrees/diploma etc regarding her age, education, marriage should be properly Apostillised for this visa requirement.
