It can very well be both, like Jack said, many times it is what we want to hear.
but where does the voice come from that is telling us something that we do not want to hear?
sometimes it is our conscience, telling us not to do something that we know is wrong, but want to do anyway.
but there are other times that we hear that still small voice telling us to DO something that is not in our nature, and it can be very specific.
in example: a few months ago, my wife was told in this still small voice to take $100, to the local womens shelter, while we have struggled financially for most of our lives, we have recently come to the point where we have extra money at the end of the pay period.
and that too can be explained away, however She could not sit still until she took the money there, even though it was after hours. and she beat on the door until someone finally answered.
2 weeks later she was told that, the night she had been so impressed to take the money that night, the women of the shelter had been praying because they needed $300 desperately. another person had been impressed to do the same thing the same night, and another person had brought the rest of the needed money the next morning.
I will leave it to you to decide which it was, God or a calm self awareness?

There are various explanations of what you are asking about.
a) It's just a normal part of the human brain/mind which consists of one part of the brain/mind asking a question and another part of the brain/mind giving an answer.
Some persons tend to sleep on a topic to discover in the morning they've found an answer.
Also, some cartoonists tend to show a person with an angel dressed in white on the right shoulder and a person dressed in red with a tail on the left shoulder which hints at two kinds of possible voices (aka the Santa Claus list: Has one been nice or naughty?).
The cartoonists are hinting at human decision making/ethics/the role of conscience/comparison of two lists/whatever.
b) In a few cases, the voice can be one type of the many epilepsies (TLE = Temporal Lobe Epilepsy)/now known as Complex Partial seizures/whatever (field of neurology).

d) What you are talking about is a theme of the Psalms of the Old Testament. That's why some persons tend to feel that God can speak to them through the Psalms of King David.
God/the idea of God and calm self-awareness/the idea of self-awareness bubble up through the Psalms so to speak.
Jesus Christ also used a similar technique or speaking approach in the New Testament (my view).
Let the weak say, I am strong.
~ Let the uncalm say, I am calm.

If it's the still small voice, it's the Holy Ghost (or Holy Spirit). If it's another type of voice, it's a not-so-holy spirit.
It is possible to hear the still small voice and not be particularly active in your Christianity. I believed but had never read a verse in the Bible when I was saved by a still small voice.
Another time I was saved by a loud command to wait at a green light as an 18-wheeler shot a red, but it was also still and small.

God says to us, Be still and listen! It is in the quiet, when He has our undivided attention, that we hear His voice. I have learned, I can also hear His laughter, in the state of my confusion sometimes too! lol! I was not there to witness Elijah's experience, but God is within each of us, in our hearts. We must always trust in the Lord God, in all things, no matter how great or small.

The still small voice is promised in the Word of God , and will always be in agreement with His revealed, completed Word. Only those who have come to God on HIS terms, by GRACE thru faith in the finished work of Christ, will hear anything from God.
The Bible tells us that "the natural man doesnt understand spiritual things because they are spiritually discerned." One has to have the Spirit of God dwelling within in order to KNOW HIM.

Awareness of the self leads to awareness of the "spark of the divine" within us. Call it what you want, but the religions of the world are united in understanding that the path to God(dess), the One, Brahman begins by looking inwards.

If you communicate w/ God (through prayer) Aren't you most likely to hear that "still small voice"? I would think so.

Jesus says, "My sheep knows [hears] my voice."
Those who belong to God, i.e. Jesus' sheep, will be able to tell the difference between the voice of the Good Shepherd & the voice of the hireling, or the bad shepherd.
The 'still small voice' is the voice of the Good Shepherd.
The calm self-awareness is the voice of the hireling.

I think anybody with kind, loving, sincere heart can experience this.
Its come as part of human nature, and usually those experience happened when people use their "heart" to "think" not their brain.
The more someone givein to their "heart" the more they get guidance.
