Okay...now I'm okay with English but I don't know if what I did was any kind of official format. I changed up some of the wording. For example, if the document is official I would not use "Sincerely Yours". That would be for more romantic or informal letters. I took out Dear, because contrary to popular belief, not ALL letters begin with "Dear". Official letters, especially requesting overdue payments and such, would require just the name of the person and a colon following after. I did take out the "Most of December" part because if it turns into a legal issue it is best to specify dates as you've had in the rest of the document. Finally I took out the word "Immediately" and "anticipated", because one makes your request for payment sound too demanding which can be okay, but its rebuttled by the word "anticipated" which makes it sound like you're hoping for a payment follow through. Try to stay neutral with your writing when requesting payment, especially if its a first notice. That's all I have. If it a was sample that you had to correct, then none of what was written was your fault anyway :D.
