Numbers one and three are incorrect both in grammar and syntax. Numbers two and four are correct. Those two sentences mean "I did something for you, was it helpful to you?" or "I did something for you, did it help you?".

Your grammar is perfect and your punctuation is pretty good too, except that I think you accidentally hit the period key instead of the comma key at the end of the first phrase of the first sentence. And you forgot to put a period at the end of the second sentence.

the correct sentences are no.2 and 4. no.2 means something that someone did was helpful/useful or not. No.4 means actually the same with different form of sentence. And no.1 and 3 are formally wrong sentence.

1 is not correct.
2- was that helpful?
it is asking if you were helped by the action. If you don't understand something and then someone helps you. They say- "was that helpful?" for example
3 is not correct
4 is the same as 2

it should be: 'what meanings do THESE questions have?' and 'i've heard a lot of these sentences a lot. and am confused.' :)

1) was that helped? -> i think the person is asking if you got help for 'that'
2) was that helpful? -> the person is asking if what he or she said was of use, or of help
3) was that help? -> i cant rephrase this question
4) did that help? -> was what he or she said helpful or of use to you

All asking the same point; was the "thing" done of benefit, did it help or not?

All past tense also.
