If I try to run an AV adapter, the sound still comes from my TV. Is there an adapter I can use that will work?

You can't.
Headphones outputs have an internal switch that turns off sound to speakers.
AV outlets are designed to drive the input of an AV receiver, not headphones.
So, get a cheap AV receiver with headphone output. (Some VCRs may heave headphone outputs)

unless you plug the headphones to one of those 13inch tvs (small black and white ones) the only way to do this would be if you hooked up wireless headphones to the back on the set (by the tv sound out [of left and right channels])

otherwise just using regular (or any other) headphones is not possible

Yes. You need a coaxial stereo female to RCA male adapter. If Radio Shack doesn't have them, get Online and inquire.
