Paying US taxes on interest from CDs?

I've made a fair amount of income in interest from CDs. I didn't make any quarterly payments so I want to start doing this starting this quarter knowing it's due on Jan 15th. I went to the IRS web site and they have a 1040ES form. Do I need to send in the whole form or can I just send in the payment voucher along with a payment?

Fill in the voucher at the bottom of the page and cut it where indicated. Send the voucher in with your check. Be careful to write your SSN and "2007 1040-ES" on the check.

Just send the payment voucher with your payment.
