honestly, why are you so unhappy with yourself? You can lose a little weight and tweak some things here and there, but you have quite a long list.
Eat fresh veggies, lay off of the chips, candies. Watch how much bread, pasta & rice you eat. Drink water, cokes & fruit juice is very high in sugar. Fruit juice is good for you, but don't overdrink it due to sugar. Milk is good for you, but ask your parents to switch to 2 percent or viatmin D. Please DO NOT give up on eating any 1 particular food. Your body needs balance, you are still a child, no matter how grown you feel. Your body needs some out of ALL the food groups.
As far as your face goes, you probably still have baby fat, you are much to young to be so worried about all of this, really. Get an MP3 player and takes walks, then start jogging, or riding bikes, or skating, stay active! Minimize your TV time to your favorite shows and make it a point to be outside doing something in your other free time.
As far as anything else, time will fix it. As I said before you are still a child, you are still growing and you will be surprised how your body shape, face, legs & arms will change in the next 5 years, you cannot rush these things.
P.S. Very few women look good "boney", what you want is "healthy"

Look up a height/weight chart for your age to see how much you should weight before you start to diet. The only healthy way to lose weight is exercise and portion control, along with cutting out soda, junk food and empty calories. Concentrate on lean meat, skim milk, fruits and vegetables plus whole grains. You can expect to lose weight at the rate of 1-2 pounds weekly, if you also add at least 30 minutes of cardio every day. This could include walking, jogging, skating, riding a bik, swimming etc. It may not wise to start a diet now, since you may be tempted to over eat during the holiday, and some people feel that they have failed competely if they over-eat even once. Just start to cut down on portions right now and add some exercise. Focus on losing the first 5 pounds and go from there.

Hey!! Its great that you wanna get healhy!!
Leg exercises: Start jogging! Around ur neighborhood or at a local park.
Arms: Push ups with get you great arms. Start small, do about 25 a day then move it up to 50 then add 25 each day, so that eventually you'll be doing 4 sets of 25 push-ups. ( do the 'girl' version of push ups)
Stomach: Do crunches!! the same number as you do push-ups.
Stores to shop at: Target has good prices, Walmart has great prices, Kohls is always having some kind of sale.
Make-up: Go easy on the make-up, too much looks fake, id try some tinted moisturizer, it'll cover up the dots and make your skin look nice and even, you could also try some covergirl powder, it comes in a green compact and you just have to find the right shade for your skin tone!
