Do the details of medical insurance claims come back to my immediate managment? Could it reflect on my character to the point they make the assumption that I am irresposible or sickly?

The nature of your conditions are protected by HIPPA. However, if you have a history of Worker's Compensation Claims, that could be made known to the management. Not neccessarily the nature of the condition, unless it impacted on your job performance. Of course, If you have repetitive absences from work, it is your employer's perogative to request a physician note. However, as health care providers we should never put anything in a note except, that you were seen and evaluated, and the length of incapacitation.

An example would be:

John Smith was seen in clinic today (insert date) and evaluated for a health concern. Based on his/her evaluation he/she should be able to return to work on (insert date). Work limitations imposed by the condition are: (insert limitations) they will need to be followed until (insert date)

SIncerely,

Your insurance can send copies of your claims to the company human resources department if they pay them, but not the conditions, just date of service, type of service (ie: office visit), amount charged, amount allowed and amount paid.

No. Due to HIPAA regulations, any information related to your health care is private and can only be shared if it is necessary for treatment or billing, or if you fill out a form to permit certain information to be shared with a specific person or organization.
