My state doesn't have any toll roads but what I've seen is D. They were this way in some other states.

Different states or authorities sign in different manners.

Usually, the first sign notes "Toll Plaza Ahead" and distance.
Approaching speeds can be on separate signs, and decrease as you near the plaza.
If posted, the cost is usually one sign, fairly close to the plaza.

This depends on the Toll Road. I have seen some, like in Oklahoma, and Illinois that inform the driver (D). Then there are some that are only (E). So if this is a question on a test, I would answer (E), because all Tolls have (E).
