Head it with the name of the action and the Court reference number which should go in the top right hand corner.

Number each paragraph and note each document you refer to with a letter or number and put the documents you refer to in that number order. The idea is to make it as clear as possible tto those reading it and as easy as possible to find any documents you refer to: Date and sign it with your address or businessa ddress on it
