Well, seeing as how a root canal is a procedure your dentist does, the cause of NEEDING a root canal would be bad teeth, lots of cavities, the need to replace fillings too much, or even disease or decay in the tooth.
When you are getting a root canal done, you are actually getting the roots of your teeth pulled out (the nerves) so that you can't feel any pain in your teeth anymore. Then, instead of a normal filling, you'll usually get what's called a crown filling, which covers more area, and also costs more.

A root canal is needed when the root of a tooth becomes inflamed, the procedure in effect kills the root thereby saving the tooth. If the tooth is pulled while inflamed (infected) the root can break off and spread the infection through out the body. This has been shown to cause death in some instances, but mostly when antibiotics are unavailable. My advise is get the root canal and save your tooth, after all you only have 32 (i hope) and won't have them that long if you do not take care of them while you can.

I believe that one of the major causes of needing a root canal are dentists. When they do a filing sometimes they drill too deep and damage the nerve. I don't know if this is unavoidable or not but it seems to happen quite often.
