Start with an apology for putting the friendship on ice. Then, ask for continued friendship or the date you seem to want.

Depending on the length of time since your break, she probably won't feel awkward anymore. If you hurt her, she may still feel scorned. If you have a mutual friend, try getting in touch that way. You won't have to be too personal right off the bat. Or if you know she will be at an event of some kind, try to make it casual.

.You probably did the right thing while dating that girl. Women always bring up the "cheating" card if you have a friend of the opposite sex. If you tell your old friend just that, I'm sure she'll more than understand why you stopped talking with her. Even knowing that cheating was not in the equation

Why not just talk to her. Just be like "Hey, long time no talk, whats been up?" Use that kind of approach to get your foot in the door to at least start chatting with her again. Tell her whats up with you, find out whats up with her, and then take it from there.

well.... i would just start up a conversation. casually. if she responds, then you're good. if she blows you off, then you should immediately apologize for being so distant but it was what you thought you should do at the time. (when in doubt, apologize, and be sincere. and apologize for something you are sorry for, even if it's not entirely your fault). tell her she is someone special to you and you wouldn't want to lose her as a friend.
