I have little red bumps and acne on my back for the past ten years or so, off and on that itchy like crazy. I'll have my husband scratch for me or I'll use a back scratcher. Sometimes my husband will say "don't you wash your back?" Yes, I do. I saw the doctor for it right after it started, ten years ago, and he recommended using soaps and washes, to include laundry soap, that are free of dyes and perfumes. I did that and it didn't help. What could be going on for this long?

You need to see a dermatologist. I have not personally seen your back to know the answer but go see a dermatologist. Leave it to the professionals.

It may be eczema. Take Hyrdocortizone and mix it with creamy petroleum jelly or a thick moisturizing cream works well for eczema or try Shea butter soap and lotion. My son suffers from that and itches and scratch until I put cream on it. If you haven't already see a dermatologist they can tell you more about your skin then your regular MD. I hope this helps! (sometimes I just talk to the pharmacist and let them know what my symptoms are and ask them to recommend something over the counter. They can even mix something up for you to try for free.)

I am no doctor but my wife was diagnosed with Keratosis Pilaris (KP). It's a type of eczema. Completely benign but common.
Dermatologist can help calm it down but it never goes away completely.
Check the link...the pictures usually show a real bad case but you will get the point.
If it is KP, the doctor will suggest either OTC lotion like, Eucerine Plus with Alpha Hydroxy Acid or something RX like Salex, also with AHA. KP is caused by excessive dry skin. One good suggestion is to put lotion on immediately after showering, even before you dry off to lock in the moisture.
Good luck

First use a loofah sponge or body scrub on your back ( get hubby to help if you need him) than use a very good moisturizer morning and night for a week. Then you can use it once a day.

Most breakouts are due to dry skin and not dirt like people think.

If you want to go cheap than use a Eucerine cream (get it at walmart) or any cream that says for severely dry skin that has no perfumes.

Good luck!
