There is a fixed fare. It's about $45 to mid-town. There is a train to mid-town but it takes over an hour.

It's a flat $45 plus tolls. But there are several airport shuttle services that are just as good. They cost about $12/one way or $20/round trip, and leave about every 20-30 minutes. The one I used dropped me off at Grand Central.

yea, a cab ride from JFK to Midtown cost $45 bucks. if i were you i would subtract $7 from that $45 and take the Airtrain to Jamaica Station or Howard Beach and change for the subway instead.
check the links below for further details.

I recently used Moon Light Limo for my wedding a few weeks ago, they were great everything was perfect, they were on time, driver was perfect and courteous and all around no problems whatsoever. They also drove us to the airport on our honeymoon, our plane was at 8 am international flight and we ordered the car for 5 am the drive showed up 15 mins earlier and helped us with the bags and the ride was very pleasent.
They also picked us up on the way back it was all around perfect experience.this is my new cab company for all my future travel needs.
Their phone number is 201 254 0136... highly recommend.
