What year are the w2 form you lost?
Call IRS at 1-800-829-1040, 1-800-829-0922 and ask for your w-2, they will send them to you for free. The years they have available are for the tax years: 2002, 2003, 2004, 2005 and 2006.
They will provide you with copies within 10 days from the date you call.

Generally Block does not have copies of your W-2 accessible to give you a copy. The easiest way to get a copy is to just ask your employer for another copy. Failing that, you can get a transcript from the IRS and state info from your state taxing authority.
When Block does your taxes, they staple your copies of documents (including W-2) in a folder behind your tax return. This is a very important set of documents - take care of it.

Call the IRS at 1-800-829-1040 and ask for an income transcript. W-2 info is kept online for 6 years. (You can also send in form 4506-T.) The information is what's necessary for federal taxes--name of employer, wages, federal taxes withheld, but NOT the state information.
(Don't use the 800-829-0922 number--that's the number for people who need to make payment arrangements for their back taxes. 1-800-829-1040 is the general information line.)
