first off, don't get a mental hang-up about it =) Take it in stride and put aside your pride for a little while.
Do positions where she can touch herself at the same time you penetrate her...that might help ;p

good question, getting caught fake an orgasms is a major blow to him as a man who is trying to please the woman he love. If it not his performance or not willing to try something new to make you reach an orgasms, then where is the problem lies? Some guy it won't bother them because they're getting their and it up to her to do the same. On the other hand it will mental haunt him that he can't please a woman in bed.
You can't imagine the damage he will suffer if he meet another woman and she find that he can be a great guy in other department but when it come to the bedroom, this where he will tuck his tail and hide. What will she think of him?
The best thing is to talk about it, try more often in different position, maybe talk dirty too you, have a few drinks to losen things up a bit. Maybe watch adult movie together and see if that can assits the effort.

Talk to her, and find out what she likes so you can push those buttons for her to ensure she hits it. Invest a good amount of time "down south" before you even get in there, if you know what I mean. Foreplay is very important to the overall experience :)

In my personal experience, I reacted quite badly, I was upset with myself because I couldn't do that to her, and evetually the whole 'sex' things drove us apart and she went and found someone who could please her.
From experience, and hind sight I would say try and work it out together, talk to her to make sure she understands before it affects your realtionship, work through the problem together.
