Food gone bad
Unwashed foods
Cross contamination
This list really could just go on and on..Unless you know the source it will be hard to determine how he got it.

Pay close attention to expiration dates on anything you purchase at the grocery store and eat. If you stock up on food as I do, freeze meat and anything that requires to be in the freezer immediately when you get it home from the grocery store. Label it and put the date that you put it in the freezer. It's a good idea to not go over 6 months in the freezer. After 6 months there is the chance of "Freezer Burn". If you defrost anything never ever re-freeze it, unless, you cook it, then it can be put back into the freezer. Be sure to re-label with the new date. Ptomaine poisoning is terrible. I had it twice from two well known restaurants. Each time it was due to bad meat.

Bacteria is the key to food poisoning.
They grow on food and counter tops and we must keep our kitchens clean and never store food longer than three days four days tops.
Milk and eggs are fast decomposing elemnts that bacteria loves to get on.
If you leave potato salad out that has mayonaise on it and let it get warm for 12 hours, you will simply be amazed at how much bacteria is already on it. Then you stick it in the frigerator for three days and then pull it out and eat some. You will be sick as hell because all that time bacteria has already been growing at a fast rate because it ewas left out for 12 hours in the warm air before you put the cold on it.
The bacteria still grow in the cold, bvut if you put in in the frige right away after you make it you will slow down the growth and make consumable for at least three days.

It's hard to predict. Sometimes, if food just doesn't taste quite right, you should probably not eat it. When eating chicken or any poultry, take a quick look at the meat when you cut into it to make sure it's not undercooked. Make sure that when cooking, that everything is clean and that you don't use utensils that handled raw food to cook other food.

*trow away old food that has been sitting in the fridge for 3 days
*fish can only be eaten once .. do not save it in the fridge for more that 2 days or do not leave it out of the fridge for mkore than 2 hours
*meat has to be well cooked and uncooked meat should remain in the fridge until ready to cook or not it spoils , smell bad and tastes nasty
meat should not be left out on the table for more than two hours or else it gets a bacteria that can be deadly
milk or cheese or any thing with milk should not be left out of the fridge for like 3 hours the most

well if you cook at home, then i think you need to get a food thermometer, with that you can see if they are fully cooked. make sure that all your ingredient are clean and not expired I think if you eat out, you might be able to check out a local news paper or food magazine, b/c where I'm from the channel 4 news makes a series that shows the cleanest and dirtiest restaurants in our area.
