You would flip it just like you normally would for an over easy egg. once flipped cook it 1-2 minutes. That should make it over medium like you want.

You can also do this...instead of flipping the egg, put a lid over your skillet. The result will look just like if you had flipped it. You can see the yolk turn to the likeness you need vs when you flip it you can't see the yolk and don't know if it underdone or overdone....

The white of the egg directly surrounding the yolk should just be starting to cloud up, then you know that you are ready to flip. I would also cook them over medium - med-high heat.
