Is He Cheating? Or Should I leave Him?
If you have a just reason to believe the man is cheating in the first place I'm sorry to say he probably is, its not because your not beautiful or aren't putting out, its simply because most unmarried 15-25 year olds men will cheat if given the opportunity. If he did cheat unless you don't care that he did, then you should leave him.

Flirting is a tricky tactic, but you shouldn't take it too seriously, sometimes he/she just flirts to flirt or if they do want to talk to you they will, also there is diff. types of flirting because they stare at you in class it doesnt mean they want you
How do I talk to her?
Make small talk, find a common ground, if she's in your class ask about an assignment, works with you ask about work, he/she looks nice that day simply say it

I have to say, I honestly do get tired of answering the same questions over and over again. You covered all those very well. There are very few exceptions to what you covered there, in fact I cannot believe I didn't think to do that sooner. Well have a great day, good luck
