I thank the answers but I need biological level answers.
My question is, why does an old man need a blood vessel dilator to get erection?
What is the matter with his penis, body, blood, etc?
What is going on with his chemistry or biology of his body?
Why the Sildenafil is good for that?
I hope that somebody knows it.

An erection is caused by increased blood flow to the penis. The veins of the penis become engorged and the tissue swells. If that circulation is impaired, that function is reduced.
Taking a vaso-dilator helps increase that blood flow again. Of course it works in ALL the veins, which is why a man can't also take other similar medication like nitrates for a heart condition. It would cause too much dilation and make the blood pressure drop too low in the rest of the body.

Ok, as you well know, all older men do not have to have any help in getting an erection. I worked in a nursing home and I can tell you that for sure....ha ha
Anyway, as we get older our circulation can become impaired for a number of reasons. To get and maintain an erection--the brain sends a signal and blood is forced into the penis, filling the blood vessels and thus causing an erection. If circulation is poor, then the erection is bad. Some diseases, such as PED can cause restriction in the blood vessels of many parts of the body, including the brain and I am sure the penis. A blood vessel dilator would allow for normal blood flow to the area of the body, in this case the penis.
Hope that helps...

Unfortunately no one answered the question:
Why does an old man have problems getting an erection?
What is happening with his blood?
Why does a young man get an erection easily?
Does the blood get thicker?
Why does it not circulate as smoothly as when the man is young?
They all explain what an erection is and how it works but no one answers my question.
Is there no doctor who can respond?:(
