Where can I get a record of my GMAT score? I took the test in 1994.?

Where can I get a record of my GMAT score? I took the test in 1994.?

I know GMAT scores are only good for 5 yrs for MBA applications, but I still need to get an official record.
I searched the ETS web site but did not find any GMAT reference or way to request old records.
