How do the rules/look change if I use a v-neck sweater or crew neck?
Lately I've been seeing guys leaving the shirt untucked, when this is done I'm guessing you don't wear a tie also?
Do I keep the collar of the dress shirt inside or outside of the sweater? Does this change if I'm wearing a tie?
How much of the cuff of the dress shirt should show?

you can do it how every you want.
untucked or tucked in. tucked in is more dressy, untucked to be casual
but wear the collar on the outside of the sweater.
you can wear a tie, or not, but if you want to look more casual don't tighten the tie all the way.

cuffs should show a little too.

You can do it in any combination....

Personally, I ALWAYS put my knot all the way at the neck if I wear a tie. Most of my sweaters are either V neck or round neck but large hole, so my collar always shows. Having my collar in or out is not an issue in this combination.

I think, how you want to wear your cloths has a lot to do with the environment you are wearing it in, rather than the actual cloths you are wearing. Do you want to look proper or casual? (again) Personally, if I am going through a trouble to wear a tie, I would perfer to look proper, not casual.
