Or those wooden angels. One is a woman holding a baby, my mom got that for my for my first mothers day. I think they are sold at hallmark too.

A beautiful necklace or a relgious artifact for the baby! A card telling her how happy you are and that you can't wait to be a wonderful aunt and support system for her. When my brother finally had a baby after a long time trying (ultimately had to do invitro with alot of complications) I gave him a tiny infant of prague. Our mom died 12 years ago and she used to have one. So I wrote him a letter to read when I wasn't around because I would cry. I told him how blessed he was and how happy our mother would be and what a blessing his daughter is! Make it personal. Think of something like this. Dont just rely on our suggestions! Be a blessed aunt! Congrats!
Edited: And by the way - Ive had 2 miscarriages and I treasure the gifts I got before because its all I have to remember those babies by! Dont be afraid to buy her something now.

Congratulations! I'm happy for her and for your entire family. She might enjoy keeping a diary during her pregnancy and relive every day years later.
It took me a long time to get pregnant, and I had someone buy the newspaper on the day my son was born. I wrapped it in a plastic bag, and 28 years later, we still have it. It was easy to buy the paper because he was born in the morning. My daughter was born at 11:10pm so we don't have one of the day she was born.
This gift doesn't sound that great, but it is: a book on breastfeeding. Why? Beacause the experience is unbelievably rewarding!

An empty baby scrapbook so she can begin the exciting journey in the scrapbook and so that the baby can look back on it when he or she is older.
I made one for my son and at barely 23 months old he can't stop flipping through the pages!
Plus, whenever I look back at the pictures of me pregnant with him, and the baby shower pics, and the pics of him newborn in the hospital I get so happy!
It'll be a way for your sis to combine all her best memories into one easy to access area! And it will make a great family keepsake for her and the baby!

A congratulations card with a nice heartfelt message is nice, you could make her a picture frame for the first u/s picture, a pregnancy book or magazines. Just let her know how excited and happy for her you are.

A gift certificate for a prenatal massage, for her to save for later when pregnancy becomes uncomfy, would be a wonderful gift. Wish someone had gotten one for me! LOL

They have these really neat pregnancy journals that she might enjoy filling out as she goes? Also LOTS of books! We pregnant women ( especially us first timers) like to read as much as we can!

pregnancy portrait package...so she'll always remember being pregnant and they make you look amazing which is something most pregnant woman dont feel when they start getting big.
Almost anyone who has been pregnant will tell you they wish they had these kind of pics.
