Family Planning Waiver Program/Forward Card?

Question

I just got my card for the Family Planning Waiver Program.
Does anyone know what this means? I feel stupid because I went to Family Planning and did NOT go through my insurance because I know Birth Control is cheaper through "no insurance" there. They told me I quilified for it. I wasn't really told what it intells other then i get free stuff.
I live in Wisconsin.
Can anyone give me info on this?
Does this affect my credit? Or anything "bad" since I'm getting this benifit?
Sorry I know nothing about this and any help you can give me would be great.
Also, my name is wrong on the Forward card, should I call them and get a new one?

Also, since I didn't really know what I was doing, I didn't put my 2nd job on it, nor i get commision/bonus at my main job.
Should I tell them? I did the math and still am under the amount to quilifity....which i saw is 185% of proverty amount or something like that.

I would recommend checking with the State Medicaid office as well to find out who you need to talk to about this card and what to do to get a correction for the Forward card you have. If they can't help contact the City Information line to see if they might be able to steer you to the correct office.
