Bit of a silly question !
the answer you are looking for is probably "d" but the damper can be anywhere along the shaft, it depends on the design of the driveshaft and its proximity to other parts of the vehicle such as the suspension arms,chassis and engine.

It should be located in the central area of the drive shaft. They are commonly known as Doughnuts. It is a thick rubber joint.

Impossible to answer unless you specify the type and model of vehicle.Its just like asking how long is a piece of string.Is it ABCD length.

Usually near the centre, but may be slightly off-centre to allow it to clear other items at full jounce.
