we really like each other he is coming for 3 months if we fall in love and decide to marry can he stay in the USA or would he have to leave and file? We both are confused. He is also coming in the winter time and I really would love for him to stay until summer to really see the beauty of my home town. We want to know if we don't marry can he extend his stay by maybe going to canada and back and get another 3 months. We both are older and want to make sure that we are right for each other and would like his visit extended and if we do fall in love and marry would he have to go back to the uk or can he just stay.

Since he is citizen of UK which is participating Visa Waiver Program, he won't have visa coming to US. When he doesn't have visa, the cannot extend or change the status, even after he marries you.
If you really fall in love with you, he has to go back to UK and you can file Fiance visa for him. If you really fall in love with you and get married to him while he is in US, he has to go back to UK and you can file Spouse Visa for him. Good luck!

If he is visiting using a Visa Waiver travel then he is allowed 3 months in a 12 month period. He can get round this by applying for a tourist visa or prolonged stay visa both will allow him to stay for a maximum of 12 months.
To apply for a K1, fiancee, visa it would be beneficial for him to apply from the UK, it is illegal for him to travel to the USA to marry you unless he has a K1 visa (although this is rarely up held).
