I understand why you are scared--surgery IS scary. Before you have the procedure, a nurse or the doctor should go over everything with you in the pre-op room and answer all your questions.

As you are not going in for a life-threatening surgery, I am sure your doctor finds you to be in good health, so the minimal risks associated with the procedure don't make him think you're likely to have a problem. That is good news because most surgeries are fine. The healthier you are, the better you likely will do.

I hope you've got a friend or two who will talk with you and maybe someone that will be there while you are under. It is amazing how comforting that can be.

If you believe in God, prayer helps as well. I've had a half-dozen surgeries, starting when I was still in high school, so I found all the above helped me. Will it completely remove your anxiety? Probably not. If we really want something, sometimes we have to take the risk and undergo the stress.

So long as everything is in order (you've had all your pre-surgical labs, etc.) see if you can distract yourself. Can you spend some time with friends and family? Got a pet? They can be very comforting. If you find you can "lose" yourself in something--watching a film, playing a game, etc. that can help the waiting time go more smoothly.

Think of how happy you will be after the surgery.
You will feel better about yourself.
Worrying ahead of time does not help very much.
Tell yourself, "I will not think about this until I am laying there waiting to be put to sleep.
Right now you are still at home doing your everyday things.
It is not happening right now.
When it comes time for you to be put to sleep, be sure to grab the nurse's hand or any hand you can get a hold of.
This will comfort you in the moment somewhat.
Best Wishes
I have also had a facial birth defect. I have had nine surgeries to correct it.

you will be fine. Ask the doctor for some meds to relax you before you go in for the surgery. I have gone under a few times, and actually, the recovery was the worst. The going under the knife was a breeze.

YOU WILL BE FINE, RELAX AND KNOW YOU WILL BE PAMPERED AFTER WARDS. GOOD LUCK!

There is nothing to be scared about but I know that my saying that will make no difference to how you feel.
Try to relax by doing some deep breathing, sounds nuts but it works and will help.
Remember you are doing this for yourself and to make you feel better about yourself, and you will be fine.

Ask the doctor if he can give you something for anxiety before the surgery. It will help. It will go well and you will wake up relieved that it is over.
