I love the "Mormons do this..."
Asking a Mormon means that generally Mormons should be the one to answer. A lot of people will tell you what they heard "Mormons do" but they don't know us. They don't know our hearts and our intentions. They also do not know what we are taught and why we do things, so it good that you are asking us!!
We pray to God through Jesus Christ as our mediator. Jesus Christ is our advocate with God. The bible is clear that we can only know God by knowing our Savior Jesus Christ. I often hear people outside our faith tell us what "we believe" but I know that Jesus is the Savior of the World.

I've always prayed to God and ended with that, and I wasn't always Mormon.
So don't act like they're the only church, or that everyone else prays to Jesus, cause most don't.
And being the mediator doesn't necessarily mean prayers. It could be that he is the person who decides who gets into heaven and who doesn't, same goes for the no one Will come to the Father except through Him thing.
Those scriptures are NOT talking about prayer.

You must have missed the other time I already answered a similar question. How did Jesus say we were to pray? Use the Lord's prayer in the New Testament as your example, and start off by praying to our Father in Heaven, or Heavenly Father. When Jesus Christ appeared in person to the inhabitants of the Americas, we believe that He did not stop them from praying to Him, as it was such a privilege and blessing to have the Son of God and Savior of the world with them. But it is recorded that even then Jesus Christ told His Father that He knew the people were only praying to Him then because of the physical presence of Christ Himself, indicating that the true pattern of prayer is still to pray to God, our Heavenly Father first. In this way, Jesus Christ shows that Heavenly Father still has supreme authority, and He defers to His Father as a sign of respect and love. See 3 Nephi 19:22 if you want to read how Jesus Christ deferred to His Father's authority even as the people were praying. Have a happy day, and I send you a virtual gummy bear.

When we close our prayers in the Name of Jesus Christ we are praying through Jesus Christ to our Heavenly Father ( God). He is our mediator to God.

You must be a neophite. When you accept Jesus' help, from that moment you become one with him(see Paul's dieertation on that subject in 2Corintians). Therefore, the Holy Spirit dwells in you and you can talk to God as Jesus did while on Earth. You don't need any magic word,or special code to get the prayer going. It is Jesus' spirit helping you pray. When you pray,do it in private. When in public manifest thanksgiving and praise. God hates repetition,so keep your prayers simple and to the point. Good luck on your journey.

We pray to God in the name of Jesus Christ. Isn't that using Christ as a mediator?

Is there a scripture that shows how to communicate with Jesus through prayer? I've never come across one.

It would seem to me that anyone using formalized words for prayers has not yet discovered the real person who is God. We should talk to Him as we would a friend or parent. Christ said that we should call God "Abba", which is correctly translated as "father". However it is the personal form of the word more equivalent to "Dad" or "Daddy". Christ wanted our relationship with Him to be that close.

Well then your issues isn't just with us LDS members, since many of my other Christian friends pray directly to God as well. I see him as my father, and I dont think he has a problem with his children coming to him in prayer to thank him for all of our blessings.

Mormons pray to God the Father and end the prayer in Christ's name.
Mormons don't pray to Christ.
""Father, thy will be done, and the glory be thine forever" was Christ telling God the Father that he will receive all the Glory for His work of creating us.
