He use to be a big time drug addict, but he's pretty much stopped, as far as I know. Also, he's had a sinus infection that the doctors say infected his brain. It made him see things and fall over everywhere, but he's started taking pills now. For a while he's been normal, but now he's talking to himself again. He keeps laughing and saying "Dude, cut it out." or "Alright, dude that's enough. Stop it." I'm so scared right now, and no ones home. What do I do?

My mom and brother are at work. He's been so normal lately, it just started today. I'm 16. I'm gonna go in the bathroom and lock it. My mother should be here soon. I'll just stay in there until she gets here. What do I do if he freaks out again? He stabbed our couch once with a huge butcher kinfe, then hid it under the pillows. I'm scared he's gonna do something like that again.

He might not have taken his pills, and that's worrysome. If he is known to be violent or this behaviour gets worse, go to emergency or call 911, because your brother needs helps

There is something very wrong with your brother. Where is the rest of the family? Why did the leave you alone with him? What is your age?
If you seriously fear for your life (and it sounds that you do) please lock yourself in your room or bathroom, call your family from your cell and if no resp once call 911.
This is an emergency and I am serious. You don't want to be another statistic, do you?

He is hallucinating. If he doesn't see or hear the voices/people as a problem, he should not get agitated. He needs to get on medication and see a therapist.

Ask him if he is OK...if not sure, call an ambulance...they will check him out for you.
