Not healthy at all. No matter what's in her sandwich, she's missing vital nutritional elements.
Remind her that anywhere below about 1200 calories, her body goes into starvation mode. She'll be tired, cross, unenthusiastic about doing anything that requires much effort, especially physical effort but including mental effort, too. This messes up school, especially classes where knowledge is cumulative, like math.
Why is her body doing this? Because when she's not getting enough calories, her body does everything it can to conserve them, including loss of energy.
She will lose a lot of weight if she keeps it up, but more of it will be muscle than fat. If she wants to lose fat, she's got to do it the old-fashioned way, a sensible eating plan over 1200 calories a day, plus mild daily exercise.

Are you saying that's all she eats?
If so it is NOT good. She definitely needs to eat more. This is no where near the daily guideline for calories and will not contain many nutrients etc whatsoever. Especially bad if it's white bread. Her body will be lacking so many things that it needs and she will have no energy, it will affect how she feels (mood, exhaustion etc)
She may lose a lot of weight, but then her metabolism may slow right down.
She needs some help. Get her to talk to a doctor and try to find the reason why she is not eating enough. In the meantime try, without being obvious if possible, to monitor exactly what she eats, write everything down. Also make sure that she is drinking enough.
Encourage her all you can to eat more without making her feel forced as this could worsen the problem. She needs variety and more calories!

I can understand your concern - one sandwich a day is not enough for anyone. Women need at least 1500 calories a day and it should be made up with healthy, balanced meals.
Regardless of whether or not your sister is deliberately enforcing such a strict diet, it could not be healthy for her. Losing weight quickly will leave her drained of energy, tired, irritable and periods may stop. Also, if she is or was quite overweight, she could be left with lots of loose skin.
It is a difficult situation for you to be in because the more you hassle her about it the more determined and secretive she is likely to be. So I would try to be relaxed - don't force her to eat but let her know you are worried about her and are there if she would like to talk.
If your sister does need to lose weight there are much more healthy ways of doing so - her doctor or practice nurse can advise.
You or your sister could also get in touch with the Eating Disorders Association for more specific help and advice.

I'm sorry to say but it sounds to me that your sister has an eating disorder. Humans crave food for energy, nourishment, and for taste. Her telling you that she is not hungry sounds like it is her way of trying to avoid the root of why she is not eating. Be there for her and try to speak to her about what is going on in her life, but don't mention the food at first. If you try to point out that she has an eating disorder, she will most likely get upset with you and will become defensive. Try to be sympathetic for what she may be going through. Best of luck. I know its difficult. My sister had an eating disorder, anorexia, and it was a very difficult time for our family. But she has been fine for almost 12 years now. It took a lot of listening and understanding.

Well from what you wrote It seems that, it's all she eating!!!??? Well you know as well as I do that thats totally UNhealthy. But if she is eating other things too which seems likely that she isn't. Then it is fine depending on what all she eating. I would talk to you sister I've had this problem before except it was my younger sister and apples. I talked to her and it seemed to help her. I hope I could help somewhat.

Your sister has anorexia.
She is going to lie to you about everything eats, so keep noticing what she is doing.
The anorexic brain works in funny ways -- we think we are not hurting the people we are around, but in actuality, we are hurting everyone, including ourselves.
Whether she is underweight, overweight, or average, get her to a doctor. It doesn't matter what the weight is -- if she keeps up these habits for long, she will be underweight in no time.
