Well certainly a madman. Whether it's considered an act of terrorism depends on his motive. Most definitions of terrorism require the act to be politically, religiously or ideologically motivated, otherwise it's classified as murder rather than terrorism. In his case a religious motivation would be impossible, however he could still have political or ideological motivations that he felt would be furthered by killing innocent people.

It depends on what the target was and why he did it. This is true regardless of the belief system of the bomber. The point being that various different types of people have done or attempted suicide bombings, if it is to draw attention to a cause or agenda then it is terrorism, otherwise it is insanity.
