If you're worried about what other people might think, take a step back for a moment. It's something that will stay with you, and what's "gay" today might be fine tomorrow - and vice versa.

I'd talk to an artist for input. Getting the flow of the work to go along the contours of your body - muscles, bone, movement - is pretty much the most important part. There are ways of doing both what you mention (and more) that depending on how they're done, both work. They can also give you general sketches of what any given piece would look like, and that's far better for deciding what you want than just thinking about it. All of my work needed a visual representation before I could really get a hang of it.

My husband is a tattoo artist and has been for 35 years. his advice to anyone getting a tattoo is first make sure it's something you want to live with for the rest of your life and into eternity. After you know what you want come back next week and tell me you still want it. You would be surprised how often people change their mind.

of course it wouldn't be "gay" unless you specifically intended for it to have that meaning. and the wave should be curling towards the front of your body, not away from it. you should definitely consult a custom tattoo artist in your area. not all tattooers are of equal talent and many aren't even "artists" in the first place.
chosing the right tattoo artist in my opinion is more important than chosing the tattoo itself. if you have a crappy tattoo artist, no matter what you get it will be a crappy tattoo. but if you have an amazing artist no matter what you get it is pretty much going to rock.
